We accept cash, check, VISA, MasterCard, American Express, Discover, and HSA Cards.

Copays, co-insurance, and deductibles are due at the time of service. If payment is not received on the same day,
a $50 fee will apply.

Insurance Notice

 Your insurance coverage is an agreement between you and your insurance carrier. We will bill your insurance for appointments but can not predict how each insurance company will respond. Any remaining balance after insurance has been billed is the patient’s responsibility.

Please Note: our providers are NOT accepting new patients with Medicare, Medicaid, MedAdvantage Plans,
OHP (even through traditional commercial plans like Pacific Source), Trillium, United Healthcare, Optum, Tri-Care, or Worker’s Compensation.

Please review the staff page to see if a provider is in-network with your insurance plan.

Cancellation Policy

New Patients

If you need to reschedule your first appointment, please contact us at (503) 389-1500 with at least 1 week's notice. Short-notice cancellations, rescheduling, or no-shows result in a $240 fee.

Returning Patients

If you need to reschedule an appointment, please contact us at (503) 389-1500 with at least two business days’ notice. Short-notice cancellations, reschedules, and no-shows will be charged $80.00 for a 20-minute appointment and $160.00 for a 50-minute appointment.
