Check-In Here
At least fifteen minutes prior to your telemedicine appointment please fill out your RPQ (returning patient questionnaire) in the Patient Portal. Once complete let the front desk know by completing the check-in process (see above button), or calling the office at (503)389-1500.
Check-in for your appointment on doxy.me (video tutorial provided) using Chrome, Safari, or Firefox web browsers. Your telemedicine appointment can be accessed using your computer, smartphone, or tablet.
